Discharge Medicines Review Wales

When you leave hospital with new or changed medicines, it can be a confusing time. Our Discharge Medicines Review (DMR) service is here to help you understand your medication, feel confident taking it, and stay safe at home.

Funded by NHS Wales and delivered under the Community Pharmacy Contractual Framework (CPCF), this free service is available to patients who have been discharged from hospital with new medicines or significant changes to their existing medication.

What happens during a review?

One of our pharmacists will have a confidential conversation with you about your new medicines — what they are for, how to take them, what to watch out for, and how they fit alongside anything else you already take. We can also liaise with your GP or hospital team if any concerns arise.

Who is eligible?

Any patient registered with a Welsh GP who has been recently discharged from hospital with new or amended medicines.

Walk in to your nearest Knights Pharmacy branch in Wales or ask your hospital discharge team to refer you.
